I'm using the latest Deluge 1.3.15 (Windows 10); and trying to put in my proxy details. I can enter my proxy host address and port, but I don't see boxes to enter my proxy username / password. What's doing on here?

Same issue here. Even when choosing the Socksv5 with auth, the boxes stay grayed out. This is ridiculous.
